More than 34 million people in the U.S. have diabetes by current clinical standards. One-fifth haven’t been diagnosed.* As the leader of a patient’s care team, family physicians are instrumental in identifying early interventions to effectively manage and treat this disease. Practice teams also need resources to help patients prevent diabetes and effective care coordination ensures patient-centered and high-quality care.
The Ohio Academy of Family Physicians’ (OAFP) newest EasyPI track is all about diabetes and resources curated on the page will help family physicians and their practice teams identify gaps in diabetes care and implement a performance improvement (PI) activity to fill them.

Just like the other tracks available, the diabetes track will guide you through the process of evaluating background information and available guidelines, determining how this information should drive patient care, and implementing team-based practice change to improve patient outcomes.
The resources provided in each section of the track can be used prior to and during your PI project. Choose the resources you need most. You don’t have to click on every link included. There are a variety of options to match where you might be in your quality journey. There’s something for everyone, no matter if this is your first PI activity or 30th!
After implementing your chosen PI initiative, resources on the page provide step-by-step instructions on how to submit your work to the American Board of Family Medicine (ABFM) through its Self-Directed Performance Improvement Activity pathway. Self-directed PI projects fulfill an ABFM PI requirement, add 20 ABFM points to your portfolio, and equate to 20 continuing medical education credits through the American Academy of Family Physicians (AAFP).
