The Ohio Type 2 Diabetes Learning Collaborative is seeking 15 -20 primary care practices (PCPs) to participate in a quality improvement (QI) initiative. This project will work to assess, screen, and treat women of childbearing age at high risk for type 2 diabetes (T2DM), with a specific focus on diabetes prevention in asymptomatic women with risk factors such as a history of gestational diabetes (GDM.) This project is funded by the Ohio Department of Health and the Ohio Department of Medicaid, and administered by The Ohio Colleges of Medicine Government Resource Center.
Work on this project has occurred for several years with specialty providers around the state, this phase of the project builds on evidence-informed strategies that have been successfully implemented within PCPs across Ohio. Sites will be engaged to participate in this QI project beginning in February 2020 and continuing through February 2021.
- Assemble a core QI group to attend monthly calls to share best practices while learning from peers
- Implement a clinical toolkit and disseminate free educational consumer resources
- Participate in regular data collection activities
- Commit to testing and adapting interventions using Plan-Do-Study-Act (PDSA) cycles related to identifying, screening, and managing treatment plans for women at high risk for T2DM
- Engage in monthly site-specific QI coaching opportunities.
We estimate that all activities will take about 4-5 hours per month among all team members and seek to integrate the QI activities into daily process flow to limit extra work on the teams.
- Improved processes for assessing and identifying women of child bearing age at risk for T2DM
- Guidance/tools on implementing proven strategies for increasing screening rates for T2DM
- Enhanced treatment protocols for providing clinical care to women during their interconception period of care, with an emphasis on treatment protocols specific to women with a history of GDM
- Access to complimentary QI resources, including a QI practice coach
- Increased care coordination leading to better health outcomes and reduce cost of care
- Credits for continuing medical education*
- Honorarium provided for data collection efforts.
