The Ohio Academy of Family Physicians has joined forces with the Ohio Health Care Quality Improvement Collaborative, a partnership of quality and health care-focused organizations dedicated to improving health care delivery across Ohio. The other member organizations include the Health Services Advisory Group, the Ohio Health Information Partnership, the Ohio Department of Health, the Ohio Department of Aging, the Ohio Hospital Association, and the Ohio Pharmacists Association.
The focus of the collaborative is to connect services between organizations on issues such as diabetes prevention and self-management, health care-associated infections, behavioral health, population health, value-based payment, and other topics. The collaborative will meet twice a year, but member organizations are encouraged to partner on programs or services on an ongoing basis to advance an effective coordination of health delivery in Ohio.
Director of Performance Improvement Erin Jech and Deputy Executive Vice President Kate Mahler, CAE, will represent the OAFP in this collaborative effort.
