The Centers for Disease Control and Prevention states that cardiovascular disease, including heart disease and stroke, is the leading cause of death in the United States. Every day, 2,200 people die from cardiovascular diseases—that’s nearly 800,000 Americans each year. High blood pressure is one of the leading causes of heart disease and stroke. One in three U.S. adults has high blood pressure, and half of these individuals do not have their condition under control.
Hypertension is the most treatable type of cardiovascular disease, and simple interventions can help you to improve the care you provide to your patients diagnosed with high blood pressure. Now more than ever, family physicians and their care teams have the opportunity to improve hypertension control rates across Ohio.
Join us at the 2018 Quality Improvement Summit on Saturday, April 7, at the Hilton Columbus at Easton to learn how you and your care team can improve hypertension control rates in your practice.
Teams will have the opportunity to pick from one of the following four topic tracks: Hypertension Management, Colorectal Cancer Screening Improvement, Diabetes Prevention and Management, or Breast and Cervical Cancer Prevention and Early Detection.
Teams will hear from expert speakers in their chosen topic track on best practices in screening protocols, up-to-date guidelines to follow, and the importance of quality improvement (QI) in patient care and population health as a whole.
In addition to the team training experience on Saturday, April 7, participants will also utilize online modules that have been custom developed for each of the four clinical topics. By utilizing the module, practices will be able to measure their team engagement, benchmark patient data, select a pathway for care delivery intervention, and re-measure outcome data after a five-month change period. The module will systematically guide participants through a Plan, Do, Study, Act (PDSA) cycle as a method to improve patient care and recognize practice success.
“Having done many of the ABFM activities on my own, I have been amazed at how easy it is to navigate the OAFP’s modules and appreciate all of the help the OAFP staff provides. The help you receive is a huge time saver and it makes you feel like part of a team instead of scrambling as an individual to complete the activity on your own. Plus, the OAFP staff reports credit to the American Board of Family Medicine (ABFM) and the American Academy of Family Physicians (AAFP) for you!” – said Terry Wagner, DO, past team champion.
More information on the Summit and each individual track can be found on the OAFP website.
- Free registration for up to four members of the care team to attend the Summit
- Travel reimbursement for each team member attending the Summit
- AAFP Live Prescribed CME credits for participation in the Summit (5 CME credits). Other members of the care team such as physician assistants, registered nurses, and advanced practice nurses can all claim continuing education credit too.
- A $250 stipend for each team that completes the chosen topic’s online module and any
additional data collection requirements - After completion of the chosen module, physicians will earn ABFM Family Medicine Certification Performance Improvement Activity credit and an additional 20 AAFP CME credits.
