With an ever-changing medical community, it’s important to remain competitive as an independent primary care physician. My practice, Advanced Internal Medicine, has been serving patients for more than three years with a focus on individualized, comfort care. We have two competitive hospitals in our town that employ most of the specialists and physicians in the area. But, there is still a good number of independent primary care physicians, like me, who run our own practices.
Three years ago, other independent physicians and I joined a local accountable care organization (ACO). We were excited about the opportunity to come together to offer better care to our patients, and leverage our size to compete with local hospital employment. However, we did not see significant progress in transitioning to value-based care, nor did we have a clear vision for how it could be achieved. At the end of our contract with a prior ACO, we began to research other options and decided to partner with Aledade.
Working with Aledade has been a completely different experience. Aledade addressed our local concerns and worked with us to get things right. Before Aledade, our group of physicians did not have a process for annual wellness visits, or hierarchical condition category coding. We were all trying to tackle each issue independently, without any insight into an optimal workflow. With the Aledade app, we now have actionable data that targets our highest priority patients providing us the information we need to keep them healthy.
It is possible to participate in value-based care and maintain an independent practice. As a solo physician, I was trying to figure it out on my own, and it was both hard and time consuming. Aledade provided the additional support we needed to move to the next level of value-based care. We now have a representative from their team that visits our office once a week, helping our practice stay on track, keep in touch between patient visits, and break down barriers so that we can provide the best quality health care. I am looking forward to the next three years of working with Aledade. The changes I have seen already in my practice have been unparalleled.

About Ryan Frazine, MD
Ryan Frazine, MD, is an independent primary care physician with his own practice, Advanced Internal Medicine, in Paducah, KY. Raised in Metropolis, IL, Dr. Frazine attended Tulane University in New Orleans, LA, and graduated Cum Laude with a Bachelor’s Degree in Biomedical Engineering. After graduating from Chicago’s Rush Medical College, Dr. Frazine began his internal medicine residency at Barnes-Jewish Hospital/Washington University in St. Louis, MO. Upon completion of this residency, Dr. Frazine returned to the western Kentucky area to start his own private practice.
