Ohio has been selected by the Centers for Medicare and Medicaid Services (CMS) as a region in Comprehensive Primary Care Plus (CPC+). The application process is open and the deadline to apply is Thursday, September 15!
CPC+ will allow up to 5,000 primary care practices in the 14 regions to participate in this care delivery and payment redesign model. With two available tracks, you can select the model that works best for your practice. CPC+ is designed to provide payment and incentives to support high-quality, patient-centered care.
The American Academy of Family Physicians is collaborating with Caravan Health to provide you with technical assistance to position your practice with CPC+ and to help with your application. Caravan Health can help you qualify for Track 1 or Track 2 of CPC+ and walk you through the application process.
