Nutrition discharge plans are a critical part of the hospital discharge process and their success relies on the involvement of primary care professionals. Many patients, up to 50%, may enter the hospital at risk for malnutrition or already malnourished and their nutrition health can further decline during hospitalization.
Malnourished patients have a 54% higher likelihood of 30-day readmissions compared to well-nourished patients. Therefore, it is essential that nutrition discharge plans are clearly communicated to primary care professionals and that patients and families are connected with local community nutrition resources/services. Post discharge nutrition interventions can include dietary modifications, home delivered meals, oral nutrition supplements, and/or follow-up appointments with a registered dietitian nutritionist.
