The Ohio Academy of Family Physicians (OAFP) is excited to have the Centering Healthcare Institute (CHI) as our Partner in Health!
The CHI is a national non-profit organization with a mission to improve health, transform care, and disrupt inequitable systems through the Centering group model. This model combines health assessment, interactive learning, and community-building activities to support positive health behaviors, ultimately driving better health outcomes.
The Centering model of care transforms traditional healthcare delivery by bringing patients and their healthcare team out of the exam room and into a comfortable group setting. CenteringPregnancy® sets interactive learning and community-building within a clinical framework, leading to better health outcomes for all. Participants in CenteringPregnancy may also have the opportunity to continue group care with CenteringParenting®, a transformative model of group pediatric care. Similarly, the CenteringHealthcare® model provides a structure for practices delivering group care to patients with chronic conditions such as asthma, diabetes, opioid addiction recovery, cancer survivors, chronic pain, and more.
The CHI is committed to offering enhanced support to the state of Ohio with the addition of a state-specific advisor, annual consortiums, and regular community of practice events. Clinical practices offering CenteringPregnancy in Ohio qualify for increased billing rates under the updated codes for group pregnancy education, released by the Ohio Department of Medicaid.
