During the COVID-19 pandemic, community organizations across Ohio continue to serve on the front lines every day to help those in need in their local communities.
The American Academy of Family Physicians’ (AAFP) Neighborhood Navigator is a public facing, interactive tool that connects patients with point of care resources and programs in their own neighborhoods. This tool lists over 40,000 social services by zip code, including: food, housing, transportation, employment aid, and much more. Many of these tools have been updated to reflect the challenges that your patients are facing because of the COVID-19 pandemic.
Additionally, Buckeye Health Plan recently asked its community partner network what its organizations are doing to help the community during the COVID-19 pandemic. Partners across the state shared creative ideas and best practices related to direct outreach during these challenging times.
