Hypertension is a leading contributor to heart disease and stroke, the top causes of death in the United States, yet it is also one of the most preventable and manageable chronic conditions when detected early and treated effectively.
Routine blood pressure measurement does more than establish a baseline, it prevents devastating cardiovascular events by identifying high blood pressure before it causes silent, irreversible damage to blood vessels and vital organs. Despite this, millions of adults with hypertension remain undiagnosed or uncontrolled, meaning countless heart attacks, strokes, and kidney complications could be avoided through timely clinical intervention.
Family physicians play a critical role in closing this care gap. By routinely screening patients, implementing evidence-based lifestyle and pharmacological therapies, and supporting self-measured blood pressure monitoring, primary care teams can dramatically improve long-term outcomes.
New EasyPI Track: Hypertension
To support clinical teams in improving blood pressure control rates, the Ohio Academy of Family Physicians (OAFP) has launched a new EasyPI track focused on hypertension management.
- Identify patients with uncontrolled or undocumented hypertension
- Implement evidence-based clinical guidelines and treatment protocols
- Track and improve blood pressure control performance within the practice
- Close chronic care gaps through structured, data-driven quality improvement.
The hypertension track is sponsored by AstraZeneca Medical Affairs – CVRM, a proud Partner in Health of the OAFP.
