The American Academy of Family Physicians has created a comprehensive Risk-Stratified Care Management Rubric (RSCM) to help you identify your patients’ risk category and is offering the rubric free as a benefit of membership. In a few simple steps, you can download and print a full-color PDF in legal- or poster-sized format as an aid to guide you through your risk-stratified care management process.
The RSCM is a framework designed to guide the physician and the care team through the process of stratifying patients into six risk based levels on health severity, social determinates, and utilization of services.
Created by AAFP subject matter experts, the rubric provides a framework on how to identify and assign patient’s health risk level; given that, provides care plan suggestions, and includes a diabetes example case illustrating different risk-levels and associated care plan suggestions.
Practices selected for the Comprehensive Primary Care Plus (CPC+) program are required to risk stratify their patient population; this resource will help accomplish that goal.
