A comprehensive patient-centered medical home (PCMH) resource for patients and physician teams is available on the OAFP website for practice distribution. The tool was developed by the Ohio Patient-Centered Primary Care Collaborative (OPCPCC) Patient Engagement Learning Center as a mechanism to better explain the hallmarks and importance of patient-centered primary care. Links in the toolbox include handouts describing PCMH in layman terms, trackers, and checklists for enhanced patient engagement and tools that improve practice competencies.
