On May 1, Ohio Academy of Family Physicians (OAFP) President Mike Sevilla, MD, FAAFP, submitted a letter to the Center for Medicare and Medicaid Innovation (CMMI) urging them to develop solutions to help Comprehensive Primary Care Plus (CPC+) practices successfully transition into other alternative payment models.
At this time, there is no clear mechanism to help practices sustain their current practice models when the CPC+ model sunsets at the end of the year. This financial cliff will result in the loss of staff and jeopardize practices’ abilities to provide ongoing care coordination and care management services for patients and communities.
The letter asks that CMMI recognize practices’ commitment to advancing Medicare’s transition to value-based care by creating a bridge for practices transitioning from CPC+ to their next payment model of choice, whether that be another CMMI model, such as Primary Care First or Direct Contracting, or joining an accountable care organization. This bridge will provide practices with the flexibility and financial security needed to continue to provide enhanced primary care services without disruption.
The American Academy of Family Physicians and American College of Physicians have partnered to create a sign-on letter for practices involved in CPC+ to self-advocate for these needed changes. CPC+ practices participating in the model are encouraged to sign-on to the letter using this form no later than Thursday, May 13. Any questions about the letter can be directed to Kate Freeman.
