The Ohio Academy of Family Physicians is now accepting nominations for the 2016 Family Physician and Family Medicine Educator of the Year awards.
These awards recognize family physicians and family medicine teachers who have:
- Exhibited extraordinary,beyond-the-call-of-duty merit
- Articulated and encouraged members and students to pursue the ideals of family medicine
- Conveyed these ideals to the public.
Family Physician of the Year
Eligibility Criteria:
Eligibility Criteria:
- Has 10 years post-residency (or equivalent) experience
- Is a member of the OAFP
- Is currently in practice
Selection Process Criteria:
- Is an outstanding family physician role model
- Maintains high professional standards and service orientation
- Provides high quality, family-centered, continuing health care
- Is active in the community, education, or other public affairs
Family Medicine Educator of the Year
Eligibility Criteria:
Eligibility Criteria:
- Is a member of the OAFP
- Has 10 years post-residency (or equivalent) experience
- Is actively caring for patients
Selection Process Criteria:
- Is a capable teacher of family medicine
- Is a successful education administrator or practitioner
- Is a productive researcher/scholar
- Is an inspiring leader
Nomination Guidelines
Nominations for these two awards may be made by any OAFP member, local constituent chapter, family medicine residency program, department of family medicine, or community individual by completing one of the forms below. - Nominate an individual for Family Physician of the Year
- Nominate an individual for Family Medicine Educator of the Year
Individuals who have been nominated, but not selected in the past can be nominated in subsequent years. The top three physicians who were nominated but not selected for 2015 awards automatically will be considered as candidates in 2016.
Award recipients will be honored on Saturday, August 13, during the OAFP Academy Awards Dinner, which is part of the annual Members Assembly held at the Columbus Marriott Northwest in Dublin, OH.
