The Ohio Academy of Family Physicians (OAFP) created Easy Performance Improvement (EasyPI), a peer-reviewed curated collection of online educational resources, to support family physicians in completing a self-guided PI project in practice and receive credit for the work they do.
Launching today is the newest track, EasyPI: Health Equity/Social Determinants of Health (SDOH)!
Just like the other tracks available, the health equity/SDOH track will guide you through the process of evaluating background information and available guidelines, determining how this information should drive patient care, and implementing team-based practice change to improve patient outcomes.
The resources provided in each section of the track can be used prior to and during your PI project. Choose the resources you need most. You don’t have to click on every link included. There are a variety of options to match where you might be in your quality journey. There’s something for everyone, no matter if this is your first PI activity or 30th!
After implementing your chosen PI initiative, resources on the page provide step-by-step instructions on how to submit your work to the American Board of Family Medicine (ABFM) through its Health Disparities/Equity Self-Directed Clinical Performance Improvement pathway. Self-directed PI projects fulfill an ABFM PI requirement, add 20 ABFM points to your portfolio, and equate to 20 CME credits through the American Academy of Family Physicians (AAFP).
The EasyPI: Health Equity/SDOH track is sponsored by Humana, a proud Partner in Health of the OAFP.
Note: to ensure we’re providing a tool that is helpful and meets your PI needs, we’d love to hear from you. Before you access an EasyPI track, please complete a pre-intervention survey.
After you’ve used the tools in a track and submitted a PI project for credit, please let us know how it went by completing the post-intervention survey or sharing your experience with Kaitlin McGuffie.
If you have questions about the EasyPI initiative, please contact Ms. McGuffie or call 800.742.7327.
Financial support for EasyPI was provided by the AAFP Foundation to the Family Medicine Chapter Alliance (FMCA) and is funded by members like you! Help programs like this continue to support family medicine by giving to the AAFP Foundation. Select “Chapter Grants” when making your gift online.
