The Ohio Department of Medicaid (ODM) has decided to discontinue its Primary Care Clinician (PCC) Report Card initiative effective Monday, October 1. This decision was made in order to streamline available resources and shift efforts towards the Ohio Comprehensive Primary Care (CPC) Program. The final release of PCC Report Cards to physicians and other health care professionals will take place on Friday, September 28.
Since June 2017, clinicians have been able to use the PCC Report Cards to compare their performance to that of other clinicians serving Medicaid members and take important steps to improve members’ health outcomes. Moving forward, ODM will work to make data regarding clinicians’ performance available to them through the Ohio CPC Program’s reporting mechanisms.
Ohio CPC is an investment in primary care infrastructure intended to support improved population health outcomes. Ohio CPC is a patient-centered medical home program, which is a team-based care delivery model led by a primary care practice that comprehensively manages a patient’s health needs.
