In May 2015, the Governor’s Office of Health Transformation (OHT) convened a Patient-Centered Medical Home (PCMH) Design Team to develop a payment model that financially rewards primary care practices that hold down the total cost of care by preventing disease and managing chronic conditions.
With input from 800+ stakeholders representing patients, primary care practices, and health plans, the OHT developed a PCMH payment model for adoption across Medicaid and commercial health insurance throughout Ohio. Originally, the plan was to roll out the state’s PCMH model by region over several years.
However, based on stakeholder feedback that the model should be implemented statewide as soon as possible, the OHT announced that the state’s PCMH model will be implemented statewide beginning in 2016, two years ahead of schedule.
