The deadline to submit an application for the Comprehensive Primary Care Plus (CPC+) program is Thursday, September 15! This innovative care delivery and payment redesign model will allow selected practices to receive a monthly payment for care coordination of Medicare beneficiaries and qualifies those practices as an advanced payment model within MACRA.
