
The deadline to submit an application for the Comprehensive Primary Care Plus (CPC+) program is Thursday, September 15! Use this CMS checklist and step-by-step document to help your practice prepare its application.
CPC+ will allow up to 5,000 primary care practices in the 14 selected regions to participate in an innovative care delivery and payment redesign model. Selected practices will receive an average of $15-$28 per beneficiary per month for the care coordination of Medicare beneficiaries and as an advanced payment model within MACRA!
Participation in CPC+ will get patients more involved in their care and more satisfied in the care you provide.
