Last spring, the Ohio Academy of Family Physicians and the Ohio Department of Health partnered together to develop a two-part outreach plan aimed to improve cultural sensitivity and communication skills among primary care physicians and a point-of-care patient information package for African-American men who are at risk for high blood pressure and high cholesterol. The project was branded as, “Check It. Change It. Control It. Your Heart Depends On It.” and the tools created include a physician guide, patient brochure, folder, and helpful tip-sheets.
A toolkit was created for both physicians and patients and is now available digitally on the Practice Tools and Resources page of the OAFP website. You can download and save these toolkits for future use. Please save and share them with your patients!
