November is National Diabetes Awareness Month and the Health Services Advisory Group (HSAG) invites you to join them to help others become the center of their diabetes care team. Visit the HSAG’s National Diabetes Awareness Month website to find tools and resources for providers and patients alike. This year, HSAG is placing a special focus on chronic kidney disease, a big risk for people with diabetes, by providing special materials that show how to better screen and prevent this disease complication.
Providers
HSAG offers diabetes self-management education (DSME) classes and invites you to host or refer your Medicare beneficiaries to a class led by our certified DSME trainers. HSAG staff can also train your medical or office team to become certified peer educators. Use the HSAG web badge to spread the word and access diabetes tools and resources.
