The Ohio Academy of Family Physicians is excited to announce its newest Partner in Health – Aledade!

As the health care system faces the COVID-19 pandemic, Aledade’s ACOs keep patients healthy, at home, and out of the emergency room or hospital by focusing on improving access to and delivering proactive, coordinated primary care. Furthermore, Aledade’s ACOs use advanced data analytics to gain insights on population health and conduct proactive care management for their most at-risk patients. In the most recently available Medicare results from 2018, Aledade’s Medicare ACOs reduced patient stays in skilled nursing facilities by an average of 17% and hospitalizations by an average of 6%.
To support its partners during this crisis, Aledade has procured large shipments of their most needed personal protective equipment (PPE), provided expert guidance on the rapidly changing policy and regulatory landscape, developed and distributed patient-facing COVID-19 content, and helped more than 275 practices launch telemedicine visits.
One resource physicians might find most valuable is Aledade’s telemedicine toolkit to help primary care practices that participate in its ACO’s successfully implement telehealth. Telehealth allows you to stay connected to your patients without an in-person visit by utilizing audio and video technology. This toolkit provides high-level information and recommendations to get you started with telehealth.
