The American Academy of Family Physicians Board of Directors has adopted a position paper introducing an advanced alternative payment model for primary care.
The paper, “Advanced Primary Care: A Foundational Alternative Payment Model (APM) for Delivering Patient-Centered, Longitudinal and Coordinated Care,” describes a model that has the potential to improve the health care system by putting patients at the center and then connecting all of their care.
