The American Board of Family Medicine PRIME SAN is extending a special invitation to members of the Ohio Academy of Family Physicians to attend a one-day meeting to learn more about health care practice transformation and how best to support physicians in the transition to alternative payment models.
The Centers for Medicaid and Medicare Services (CMS) leadership and their support teams will be in attendance to solicit feedback from attendees on the transformation support packages that are being developed through the Transforming Clinical Practice Initiative.
Participants will also hear from physicians who have reached “exemplary” transformation status and are ready to share their successes and discuss opportunities and barriers to spreading the transformation model.
The meeting will be held on Friday, November 9, at the Capital Hilton located at 1001 16th St. NW in Washington, D.C., and will be hosted by the Patient-Centered Primary Care Collaborative.
