OAFP is pleased to sponsor the following awards. Each award recipient will be formally honored at the annual Members Assembly.
Nominations must be received no later than Friday, April 6, 2012.
Family Physician of the Year
2011 Family Physician of the Year Recipient
- Is an outstanding family physician role model.
- Maintains high professional standards and service orientation.
- Provides high quality, family-centered, continuing health care.
- Is active in the community, education or other public affairs.
- Has 10 years post-residency (or equivalent) experience.
- Is a member of OAFP.
- Is currently in practice.
Family Medicine Educator of the Year
2011 Family Medicine Educator of the Year Recipient
- Is a capable teacher of family medicine.
- Is a successful education administrator or practitioner.
- Is a productive researcher/scholar.
- Is an inspiring leader.
- Is a member of OAFP.
- Has 10 years post-residency (or equivalent) experience.
- Is actively caring for patients.
Nomination Guidelines
Nominations may be made by any OAFP member, local constituent chapter, family medicine residency program or department of family medicine. To check a physician’s membership status, please call 800.742.7327 or send an e-mail to Kaitlin Smith.
Individuals who have been nominated but not selected in the past can be nominated in subsequent years. The top three physicians who were nominated but not selected for 2011 awards automatically will be considered as candidates in 2012.
Friend of Family Medicine
Award not given in 2011
- Any Ohio elected official
- Media outlets in Ohio
- Print and broadcast reporters and hosts for Ohio media outlets
- Editors for Ohio media outlets
- Publishers for Ohio media outlets
- Freelance writers who have published in Ohio newspapers or magazines
Timeframe
Any work, articles or broadcasts that represent a nominee’s support of family medicine must have been completed between Jan. 1 and Dec. 31, 2011.

Nomination Process
Complete and submit the Friend of Family Medicine Award Nomination Form along with a copy of any articles, broadcasts (date and title of story that aired is sufficient) or other examples of work that exhibit the nominee’s support of family medicine.

Nominations may be made by the following individuals who have knowledge of the nominee’s accomplishments:

Selection Process
