Corporate Membership Benefits
Your company's corporate membership means that you support OAFP Foundation's (OAFP/F) many charitable programs that encourage medical students to choose the family medicine specialty, provide seed grants for research and establish partnerships to enhance public awareness of the specialty and its messages promoting wellness. OAFP/F is the only charitable organization in Ohio that exists to improve and increase access to quality healthcare by investing in the specialty of family medicine. Family physicians provide comprehensive, continuing care to all members of a family thus contributing substantially to a healthier quality of life and valuable healthcare cost containment measures. Your company's support of OAFP/F is vital to creating a healthier Ohio for all citizens.
WHAT ARE THE BENEFITS OF YOUR CORPORATE MEMBERSHIP?
When your company elects to become a corporate member of OAFP/F, you receive a package of benefits that puts your name before Ohio Academy of Family Physician (OAFP) members as supporters of their specialty.
These benefits include:
- A subscription to the OAFP/F Cornerstones, a quarterly newsletter that carries the honor roll of corporate members and pertinent stories on member companies.
- Print recognition in publications, including the OAFP/F annual report (circ. 3,000 copies) and OAFP's The Ohio Family Physician magazine (circ. quarterly, 4,700).
- A 20 percent discount off published advertising rates in OAFP's The Ohio Family Physician magazine (circ. quarterly, 4,700).
- Recognition in electronic communications, including The Weekly Family Medicine Update e-newsletter (circ. weekly, 2,500) and on www.oafpfoundation.org (the official OAFP/F Web site).
- For enhanced community visibility, news releases sent to local newspapers about your new or renewed Corporate Membership.
- Opportunity to discuss healthcare issues of mutual concern and to design OAFP/F programs that will meet those needs, including designated sponsorship of programs and/or special events.
- The opportunity for a representative of your company to serve on OAFP/F's Board of Trustees or to serve on the Corporate Advisory Council.
- Corporate Membership placard to display at your exhibit booth (for those companies who exhibit) during OAFP educational workshops.
- Corporate Member logo for your company to use in your physician-directed publications to signify your partnership with OAFP and OAFP/F.
- Recognition for your support of OAFP/F at the Board of Trustees' Annual Recognition Program held in conjunction with the annual meeting of OAFP each August.
Corporate Member Giving Levels
Corporate members support the OAFP Foundation annually at one of the following levels:
Grand Patron Annual Membership Dues: $5,000 and above
Champion Annual Membership Dues: $2,500 - $4,999
Benefactor Annual Membership Dues: $1,000 - $2,499
Sustainer Annual Membership Dues: $500 - $999

Corporate member benefits are commensurate with the respective giving level. Contributions that are greater than $5,000 will receive additional, special recognition. The OAFP Foundation is a charitable public foundation recognized by the Internal Revenue Service as a 501(c)(3) tax-exempt organization. Contributions are tax deductible to the extent allowed by the law. The OAFP Foundation's Federal Identification Number is 31-1191776.
